Site Inventory Form

The Iowa Site Inventory Form is the standard form used by the State Historic Preservation Office to record information on an architectural or historical property.

Historical/Architectural Database (HADB) Form

The Historical Architectural Database Template only needs to be filled out if you are sending in a separate context or survey report with the Site Inventory Forms. Right-click on the link below, save to your Templates folder, and use like the Site Inventory Form above.

Black & White Photographs

The Black & White Photolog Template only needs to be filled out if you are sending negatives or slides to the SHPO. Right-click on the link below, save to your Templates folder, and use like the Site Inventory Form above.

Digital Photographs

For individuals wishing to submit digital photographs in lieu of archival black & white prints or slides for architectural, historical, and archeological survey and evaluation projects, please follow the guidelines provided in the document below.

The Digital Photolog Template should only be filled out if you are sending a CD-R with digital images to the SHPO. Right-click on the link below, save to your Templates folder, and use like the Site Inventory Form above.
